Expanded Access Program for the aged, blind and disabled, and will be
proud to serve an estimated 16,000 individuals on the islands of Kauai,

Maui and Oahu for Hawaii-licensed healthcare professionals who share our

We're looking for an RN with clinical and long-term care experience to
join our new team. As the Field Service Coordinator, you would act as an
advocate for our Medicaid members with the goal of keeping them happy
and independent. This field-based role offers an excellent opportunity to

Key responsibilities of the Field Service Coordinator include:
- Visiting members in their homes or other facilities
- Identifying early risk factors
- Conducting ongoing assessments
- Creating care plans
- Focusing on disease management and decreasing unnecessary
hospitalizations
- Consulting with the primary care physician to deliver and coordinate
necessary services
- Building relationships with members and their families
Requirements include:
- A current, unrestricted RN license in Hawaii
- Clinical experience
- A background in long-term care
- Familiarity with electronic documentation
- Basic computer proficiency and good typing skills
In addition, the following assets would be ideal:
- Case management experience strongly preferred
- Medicaid and/or Medicare experience
- Bilingual (Chinese, Filipino, Japanese, Korean, Samoan or
Vietnamese)
skills
- Experience in special needs pediatrics
- Ventilator knowledge
- Experience in behavioral health
